New patient?

Please note the following instructions for your first visit with us:

1. Bring a list of your currect medications.
2. Please do not wear any make-up, or be prepared to take your make-up off when you arrive at the office.
3. Arrive 30 minutes prior to your appointment time. You will have one more form to fill out once you get here, then we can complete your paperwork, get your chart together, and get you to the treatment room by your scheduled appointment time.
4. Consult fee is $50 regardless of insurance.
5. Bring your insurance card and drivers license.
6. If you need to reschedule or cancel your appointment with us please call with a 24 hour notice.
